"If a GS registered installer installs a new boiler that does not meet the standards of the Commissioning Checklist, but signs it off anyway, what is to be done?"

I'm not a lawyer, but to me that's 'Fraud by false representation', which is a serious criminal offence under Section 2 of the Fraud Act 2006. Report them to Trading Standards and/or the police.
 
Oh, okay, is it?

They have certainly ticked all the boxes in the last section "All Installations" but none of that is true.
I suppose it is difficult to prove that someone hasn't done something...
