If it is the same as the ones i have done (which it looks like) then it is just a case of pulling it out. Dont be too heavy handed but just keep wiggling it and pulling at the same time. I spent 40mins on one before it finally popped out - i had bought a new cartridge previously and didn't want it to be one of those things put back in the van for another day, never to be seen again. The top nut is so that the outlet / wall elbow can be fed from the top or bottom of the valve.
