This is a brand new pipe, but the access plate can't tighten as the bolt thread seems to be miscast. Any remedy or should I get co back?
 
Call them back as it`s brand new. There is a remedy but why should it cost you to make good?

Either replace the bolts if the threads are damaged which is unlikely, use shorter bolts if they are going into a blind hole and touching the bottom or some people will use a thicker gasket if there is one which is ok as long as it seals or re thread the holes.

Had the plumber replace the pipes less than 2 month from the sink that lead into a cast iron pipe. They also unblocked gunk from the start of the cast iron pipe. The gunk is in the photo I've attached. The pipe work is also attached(it's disconnected as we cleared the u bend to clear it out)...
